MCAAP Post-COVID Return to Play Protocol
Coronavirus disease (COVID-19) is an infectious disease caused by the SARS-CoV-2 virus. Most people infected with the virus will experience mild to moderate respiratory illness and recover without requiring special treatment. However, some will become seriously ill and require medical attention.
Per the Massachusetts Chapter of the American Academy of Pediatrics, all children and adolescents who test positive for COVID-19 should notify their pediatrician and the infection should be documented in their medical record. For a child or adolescent who is SARS-CoV-2–positive, even if the case is considered asymptomatic or mildly symptomatic, assessment by primary care physician (phone, telemedicine, or in-person consultation) is required so appropriate guidance can be given to the family and school. Based on the severity of symptoms of the case, the MCAAP has published Return to play after COVID-19 infection algorithm (see PDF) to guide pediatric patients through the protocol of returning to participation. This form (see "COVID-19 Return to Play Form" below) is required before returning to extracurricular athletics and should be submitted to the nurse and athletic trainer.
After a student has been diagnosed with COVID-19, they must be cleared for progression back to physical activity by an approved health care provider* (MD/DO/PA-C/NP). The student cannot begin the return to play process prior to receiving medical clearance.
*: Approved health care providers include, but are not limited to, a member of the student's Pediatric Health Care Provider Team, a Primary Care Provider-referred Cardiologist, the physician who has completed the student's pre-participation physical, or an appropriate provider approved by the nursing staff/COVID team leader. Unless otherwise specified, a health care provider from an Urgent Care, Emergency Department, or inappropriate specialty should not be completing this form.
Asymptomatic or mild symptoms (<4 days of fever >100.4, <1 week of myalgia, chills, or lethargy)
Stage 1: Normal Practice
Stage 2: Return to Competition (Athlete must complete one practice before competing)
Moderate symptoms (≥4 days of fever >100.4°F, ≥1 week of myalgia, chills, or lethargy, or a non-ICU hospital stay and no evidence of multisystem inflammatory syndrome in children (MIS-C).)
One day of symptom-free tolerating activities of daily living after 10 days have passed from symptom onset or positive test
Stage 1: One Light Practice or 30 Minutes minimum of Cardiovascular Exercise on own
Stage 2: Full Practice
Stage 3: Return to Competition (No games until after 14 days have passed from symptom onset or positive test)
Severe symptoms - ICU stay and/or intubation, or MIS-C
Restrict from exercise for 3-6 months, obtain cardiology clearance prior to resuming training or competition
Refer to pediatrician and/or cardiologist for plan to return to activities
