Instructions:
Submit only 1 LAC Plan for the whole school year
LAC Report must be submitted quarterly. Combine all monthly LAC Accomplishments in 1 file then submit only 1 report per quarter. Included in the report must be the following:
a. Activity Proposal/Design
b. LAC Accomplishment Report
c. LAC Attendance with Signature
d. Photos
e. M&E Reports
Schedule of Submission:
📝LAC Plan shall be submitted once for the school year. It should contain the whole school-year LAC Plan of the School.
📝LAC Report shall be submitted quarterly.
IMPORTANT: Make sure to use the template provided.
LAC Plan: filename format: District_NameOfSchool_LAC Plan_2024_2025 (Example: D1_KidapawanNHS_LAC Plan_2024_2025).
LAC Report: filename format: District_NameOfSchool_LAC Report_Quarter_2024_2025 (Example: D1_KidapawanNHS_Q1_LAC Report_2024_2025)
