Tryout Information and Registration Packet: Includes tryout dates, requirements, student information, emergency contacts, acknowledgments, and registration materials.
MHSAA Physical and Consent Form: A current MHSAA physical and parent consent form must be on file before a student may participate. The physical must meet the applicable date and eligibility requirements.

Team Contract: Every selected athlete and a parent or guardian must review and sign the official team contract. It includes team expectations, attendance requirements, academic standards, estimated costs, payment options, uniform responsibilities, and acknowledgment pages.
Team Schedule: Review practices, meetings, payment deadlines, the tentative Mid American day camp, and confirmed team events.
Team Information: Review the regular practice commitment, academic expectations, estimated costs, payment plan, communication procedures, and family responsibilities.

The estimated team cost is $375 per selected athlete. Families may pay the full amount at once or use the three-payment plan.

Families will indicate their payment choice on the team contract. Payments must be submitted through the school-approved collection process. Please do not send payments through an unapproved digital payment account or directly to a vendor.
A parent or guardian should contact Coach Justice privately before a deadline if an alternate school-approved arrangement may be needed.
