This project was created by the Jefferson County Communities That Care coalition, which is housed within Jefferson County Public Health through December 31st, 2022. Beginning January 1st, 2023, Jeffco CTC will be a project of the Colorado Nonprofit Development Center. This resource was developed with funding from a Coalitions Organizing for Prevention grant from the Colorado Department of Public Health and Environment and a Drug-Free Communities grant from the Centers for Disease Control. Submitting an Invoice to Jeffco CTC

Please note, starting Jan. 1, 2022, all contractors, community advisors and interns must submit an invoice in order to receive payment. There are 2 steps:

  1. Create an invoice. You may use the template below or use your own invoice form. Invoices can be word documents, google documents, spreadsheets or pdfs. You will need to look at your agreement/ contract to create your invoice.

  2. Submit an invoice by emailing it to: JeffersonCountyCTC@gmail.com with the following information:

    1. Your name (e.g. Contractor/ Community Advisor/ Intern)

      • Person, organization or business, depending on who the contract/agreement was with.

    2. Person Submitting this invoice or form

      • May be the same as above.

    3. Phone number if we have questions

    4. Today's date

    5. Amount of invoice

    6. Please bill to:

      • Drug Free Communities Grant

      • Communities Organizing for Prevention Grant

    7. After submitting this invoice, will your agreement be complete or do you intend to submit additional invoices under this agreement/MOU/contract?

      • Completed: this is the last invoice submission under the current agreement.

      • Additional invoices: the amount of compensation remaining, after this payment, should be listed below.

    8. Amount $ remaining, if applicable.

Tips for using this template:

  • Click on the template and download it, then save your version using your name and invoice as the file name MyNameInvoiceJanuary2022.

  • Put your information in the top chart-- don't assume we have that!

  • For the second chart

    • Dates can be a range of dates.

    • Description of services can be short (e.g., Part one, attend meetings).

    • If your agreement was to finish a task (such as create and present a video) put "NA" under hours.

    • Use your signed agreement, which was emailed to you when you started, to find the amounts you can invoice.
