All parents/guardians of James Hart Athletes must acknowledge that they fully understand and accept the policies and procedures for athletics at James Hart. After reading through the code of conduct, please fill out the form acknowledging you accept all policies and procedures. You will need to complete it once a school year for each athlete. 

1.) Read the James Hart of Code of Conduct.

2.) Fill out the form stating you have read the Code of Conduct and understand James Hart Athletic policies and procedures.
