CUSTOMER REGISTRATION

• Forms should be filled using black pen

• Forms should be filled in Block(capital) letters

• There should be no cancellation/correction on forms for effective data capturing by the system

SMS STATUS to 799 is now free of charge or dialling *789# to check SIM REGISTRATION STATUS

Only one form should be used in registering two lines and only one process is involved, Line two(2) should not be registered separately. However if customer owns more than two MTN lines, customer will be required to fill another form for the other lines

• Only the Left and Right Thumb print of customer should be captured

• Be courteous when interacting with customers. Use words like Please, Kindly, may I, Sir/ Ma, e.t.c

• The “Town” and “State” columns are a continuation of the customer’s contact address

National Identity Number (NIN) /NIN Slip as a Means of Identification. Please note that this is mandatory

Requirements for SIM Registration Update

Any one of these valid government issued IDs: Your NIN slip, Your Voter’s Card, Your Driver’s License, Your international passport.
