How to make an insurance claim

1. Go to n.foreignerdb.com/hufs
2. Log in using your student ID number, alien registration number, or passport number.
3. Download the insurance claim report and personal data processing agreement (on the left side of the page under Download), print them out, and fill them out with the relevant information.
4. Scan or take pictures of all relevant medical documents from the hospital or pharmacy (contact our office for specifics and examples).
5. Fill out the report and agreement and scan or take a picture of the completed documents and your ARC and bank book. Put those and your medical documents together in one .zip or PDF file and upload it on the website (Upload Documents Claims menu).
6. After a few days your claim will be processed and you will get a refund. If there are any additional documents required, you will be contacted.

Coverage

The insurance provided locally through Meritz costs approximately 60,000 (around 60$ USD) per semester. Here are the benefits of this insurance (as of Fall 2018):

1. Accidental Death and Disability: 100,000,000
2. Accident Hospitalization Coverage: 50,000,000
3. Outpatient Treatment (Accident): 250,000( maximum for one day)
4. Sickness Death: 30,000,000
5. Prescription Drugs (Accident): 50000 (maximum for one day)
6. Disease Hospitalization Medical Expenses: 50,000,000
7. Outpatient Treatment (Disease): 250,000 ( maximum for one day)
8. Prescription Drugs (Disease): 50000 (maximum for one day)
9. Liability of Compensation for Daily Life: 30,000,000
10. Special Coverage: 30,000,000
    • In case of death incurred by an accident or illness, the remains will be transported within 1 year from the date of the accident or disease occurrence and the costs of transportation, search and rescue, airfares will be refunded to the legal heir.
    • In case of disappearance, the costs of search and rescue by plane or ship will be refunded.
    • If the insured is hospitalized for over 14 days, there is a relief cost of transportation and accommodation that will be refunded. (2 guests/within 14 days)
11. Over 80% High-Disability (Disease): 30,000,000
12. Other Circumstances
