Host Family Agreement
Sexual Harassment, Molestation, Abuse Policy
Background check release form
A PH staff member will be in touch to schedule a brief home visit
*If you have hosted with us before, send us an email at host@ph-int.org with your interest and we will be in touch.
**Please note: These forms request information about the potential host family. PH program staff will conduct an in-person interview with approved candidates to complete this process. Completing these forms do not obligate you to host, nor does it ensure you will be approved to host.**
