ADMISSION PROCESS

The admission process begins when a man decides to do something different and seek help.

Should the Hope House in Athens be where the man wishes to establish his foundation in recovery, and after he has self qualified based on the checklist below, he should

As a way to speed up the admission process, a potential guest can download our Info Pack and fill it out.

Once completed the information can be sent to the program director via regular email or hand delivery. The potential guest will set down with the program director upon arrival to go over the information again, or obtain it for the first time if the guest did not complete it prior.


PROGRAM FEES

Program fees at the Hope House are $200 /wk.

A $500.00 non refundable entry fee, along with 2 weeks program fees are due upon entry.

If the incoming guest does not have current valid identification (DL or ID & SS Card or Birth Cert) sufficient to gain employment, six weeks of program fees, in addition to the entry fee, are due upon arrival.

Hope House is a Level II Facility

MINIMUM REQUIREMENTS


A Potential Guest Must -

A) Be admitted voluntarily


B) Be 18 years of age or older (If emancipated guest can be 17 years of age.)


C) Express a desire to recover from an addiction to drugs or alcohol


D) Be at least one day from the last use of alcohol and/or drugs.


E) Have the capacity for active participation in all phases of the program.


F) Have adequate control over their behavior and not be considered as dangerous to self or others.


G) Be ambulatory and meet personal needs without assistance.


H) Be medically stable. Any individual with ongoing medical problems, including dual diagnosis, may be accepted if they are addressing these problems with outside professional help.


I) Be assessed as medically appropriate and free of any illness that requires isolation from others.

J) Have adequate resources to pay for the program.


There is a $500 admission fee and a weekly fee of $200/wk. With proper, job ready valid ID, the admission fee and 2 weeks of program fees are due on arrival. Without job ready, valid ID the admission fee and 6 weeks of program fees are due on arrival.


The Hope House has 30, 60, & 90 day programs

as well as six (6) month & one (1) year.


Guests can stay up to five (5) years.

EXCLUSION CRITERIA

A Potential Guest Cannot:


A) Have outstanding, uncleared warrants issued by any law enforcement agency.


B) Have been previously dismissed from the Hope House or other recovery residence for violence, theft, or mayhem.


C) Have clinical manifestations that meet criteria for a more intense level of care (e.g.

Acutely psychotic or a danger to self or others)


D) Have severe permanent deficits in recent memory, attention, concentration, who cannot attend effectively to activities of daily living and whose cognitive impairment prevents them from understanding and participating.


E) Have ongoing medical issues, which require a more intensive level of monitoring and care than can be provided by the Hope House.


(Potential guests with Medically Assisted Treatment, for instance, require a more intensive level of monitoring and care than can be provided by the HOPE HOUSE. Therefore, the potential guest would be disqualified from the Hope house for that reason.)
