Notarized Affidavit or Unsworn Declaration: Signed by you as the supervisor, certifying that instruction will be in English, required subjects will be taught, you hold a high school diploma/GED, and no household member has been convicted of designated criminal offenses under Section 111. Philadelphia families must also attach the HSE Student Information Sheet.
Educational Objectives: An outline of proposed educational objectives by subject area (English, Math, Science, History/Civics/Geography, Safety, Health/PE, Art/Music) for each young person.
Health & Immunization Records: Proof of immunizations and required health services. Dental Exams are required in Grades 3 & 7; Medical/Physical Exams are required in Grades 6 & 11. Medical, religious, or strong moral/ethical exemptions are permitted by law.
Special Education Approval (If Applicable): If a young person has been identified as needing special education services under the Education of the Handicapped Act, the annual affidavit must include written approval of the program/objectives from a certified special education teacher or a licensed clinical/school psychologist.
Compulsory Age: Compulsory school age in PA applies to young people who turn 6 years old by September 1st and are under 18 years old by September 1st.
Document Copies: Always submit copies only, as school districts will not photocopy or return submitted documents.
Discontinuing Homeschooling: If enrolling in a public, charter, cyber, or private school, immediately notify your local district's homeschool office to close your file.
