Pursuant to CHED Memorandum Order No. 09, Series of 2013, specifically Section 14, which states that the Guidance Office shall use “an integrated approach to the development of well-functioning individuals primarily by helping them to utilize their potentials to the fullest” and in compliance with the Gender and Development (GAD) Sex-disaggregated Data, the Guidance and Counseling Center (GCC) will be collecting personal data and information from all working students (full-time, part-time, self-employed, etc.) of the institution. Hence, the data gathered/results will serve as the basis and will be used for the proposed educational guidance program for all working students in response to SDG 4: Quality Education and EDCOM II Final Reports. 2025
Pursuant to CHED Memorandum Order No. 09, Series of 2013, specifically Section 14, which states that the Guidance Office “shall maintain students’ cumulative records containing relevant information about the students (e.g., family background, test data, disability records, etc.). Records shall be appropriate, usable, and regularly updated,” the Guidance and Counseling Center (GCC) will be collecting personal data and information from all first-year students and transferees at the institution. With this, you are REQUIRED to submit your Personal Inventory Forms (PIF) on or before SEPTEMBER 30, 2025 (Tuesday). You may download the PIF via the Google Form link or scan the QR code below:

The PIF must be printed on A4 size paper (back-to-back) and handwritten using only a black or blue pen. Afterward, the class president of each block shall collect all the PIFs and turn them over to the Guidance and Counseling Center in a white long folder (one folder only for the entire class). In addition, the PIF is required for the signing of your clearance.

The Guidance and Counseling Center (GCC), in collaboration with the GABAY Organization, will spearhead PAGLAOM (Hope) ’25: Seniors’ Recollection on May 19-21, 2025. (Further details will be provided in a separate office memorandum.)
In line with this, all 4th year class presidents are requested to attend a meeting on April 16, 2025 (Wednesday), at 9:00 a.m. in Arangkada Hall to discuss important matters regarding the activity.

One of the services offered by the Guidance and Counseling Center is the Exit Interview, which aims to provide an opportunity to receive genuine, unfiltered feedback that can not only pinpoint what is working well but also shed light on less effective areas of the students’ experiences. It has been used to evaluate both academic and non-academic programs of the institution, as well as its respective units and departments. By doing so, it stimulates and improves ongoing feedback for department programs and services. Moreover, it aims to determine the level of satisfaction of graduating students with the services and support provided by academic and non-academic departments/units.
In line with this, all 4th-year graduating students of SY 2024-2025 are required to complete the Evaluation and Exit Interview Form on or before April 30, 2025 (Wednesday). Additionally, all 4th-year class presidents are REQUIRED to attend the closed-door exit interview on April 16, 2025 (Wednesday), 9:00 a.m., at the Arangkada Hall, as this is one of the requirements for their class clearance. Records shall be appropriate, usable, and regularly updated,” the Guidance and Admissions Office (GAO) will be collecting personal data and information from all enrolled students, especially new students at the institution. With this, all regular and irregular first-year students are required to submit their Personal Inventory Forms (PIF) to the Guidance and Admissions Office (GAO) on or before October 14, 2024 (Monday). Furthermore, all first-year class presidents from various programs and courses are required to attend a short meeting on October 4, 2024 (Friday) to discuss important matters related to the PIF and their clearances at the Administrative Building, Arangkada Conference Room, 2nd Floor, from 9:00-10:00 A.M. Attendance is a MUST.
