Girl Scouts is all about... forms! We like to laugh about how leaders spend WAY too much time on forms, but maybe we can also cut down the time by bookmarking the most common forms and adding some tips about when to fill them out.
Permissions and Health History Form - for the Girl Scouts
This form contains important information (like allergies, emergency contact, and medical information) that should be reviewed every year and renewed every 2.
The leader needs have these at every troop meeting and event.
Adult Health History Form
This form is for you and your adult volunteers. You don't need them for regular troop meetings, but everything else, like overnights, outings, and events will require it.
If you would like to keep this information confidential, you may opt to keep it in an envelope that's only opened in case of emergency. But please communicate allergies or medical concerns.
So you've decided to take the troop camping. Yay! It's honestly the best time to watch the girls grow and learn and stretch themselves. It's best to start your paperwork 2 months in advance. Here's what you'll need for paperwork:
All volunteers who attend need to be registered and background checked
Transportation Cards- Anyone transporting youth, other than their own child, must have this completed. It contains drivers license and insurance information.

If you'd like to have a meeting outside your normal meeting location, like a Firehouse tour or zoo.

You should submit the Short Trip Application to the Service Unit Manager (atleast 30 days before travel). If you need additional space to accommodate those attending, you may use an Excel document, like this one.
If you are staying at a non-commercial site (like an AirBNB), the rental site safety checklist is required.
Trips out of state, lasting 1-2 nights.
