Navigating physical, mental, emotional healthcare and community resources and social service systems can feel overwhelming for anyone facing life’s toughest, most loneliest, chaotic, and distressing challenges. At Fundamental Chromatic, our trained, experienced, knowledgeable, and compassionate Case Manager partners with individuals and families (collectively -- households) from all backgrounds to provide personalized support, leadership, and guidance in managing healthcare, housing, and additional-essential life needs. 

What We Do

Our dedicated Case Manager conducts detailed needs assessments to understand each of our clients' unique situations. From introductions to asking for help, offering support, and connecting clients to resources in the community and nationally, we work with our clients to develop a detailed, goal-oriented, Plan of Care that addresses clients' specific concerns, and aspirations. Leading the path to creating Goals, that the Case Manager and each client work on setting, gathering resources, sharing knowledge, and devising a plan to achieve the Goal(s). The Case Manager does all she can to provide honest, genuine, unique, and extra special - resources that are not otherwise accessible to the client. 

Fundamental Chromatic is led by trustworthy leadership, that strives to create and develop avenues where clients will experience personal-growth through considerable Case Management, and adequate supports. Fundamental Chromatic provides coordination and ongoing monitoring to make sure clients are receiving the services our clients deserve, need, value, and are willing to work very diligently to achieve, and even exceed

How We Help

Case Management services include:

• Connecting you to healthcare providers and specialists

• Advocating for your access to medical, mental health, and community services

• Assisting with Medicaid, insurance, and appointment scheduling

• Arranging transportation to appointments or community resources

• Identifying and linking you with housing resources and support

• Supporting your wellbeing with referrals to peer recovery groups and wellness programs

• Providing ongoing follow-up to monitor your progress and adapt your care plan

• Transition Services
