Behavioral Health Continuum of Care:
Recovery support:
Ongoing peer support, life skills lessons, coaching, navigation, and development, and employment assistance, coaching, and development resources to help individuals maintain stability after treatment. As well as Advocacy and Connections to Resources.
Connections & Referrals for - Treatment options:
Individual &/or group therapy, medication management, various outpatient programs depending on the severity of mental health issues, and/or Duel Diagnosis', Crisis', and need for supports. Medication Assisted Therapy (MAT Program) for Opioid utilizers (self-medicating, addiction, relies on, etc.) to help opioid utilizers get off illegal or over-utilized opioid drugs, and on a safer-regulated Medication such as Suboxone.
Connection & Referrals for - Early intervention:
Grief counseling, support groups, crisis hotlines for individuals experiencing mild symptoms. Connections to Crisis Center(s), and other Mental Health and Physical Health Care Providers.
Connections & Facilitation for - Help applying for and securing Government Benefits: Health Coverage, Nutrition Assistance, WIC, Adult Financial for families and people with Disabilities and/or elderly individuals and households. Help navigating and applying for SSI and SSDI (and follow-up)
Resource Connections & Navigation: assistance in accessing essential resources such as housing, healthcare, financial aid, education, employment, mental health services, addiction recovery programs, legal aid, and other community-based services to help individuals achieve stability and self-sufficiency.
Prevention programs:
Educational initiatives, stress management workshops & navigation, early screening for mental health concerns, peer supports, exercise programs, healthy eating and lifestyle coaching, literature and guidance, Coping Strategies and Skills.
Workshops-
Workshops and 1on1 Appointments for learning how to navigate, schedule, and manage Health First Colorado covered Non-Emergent Medical Transportation Services. (Scheduling and Cancelling Rides to Medical Appointments. As well as setting up Online Scheduling Application Access. Help Initiating the Need for Transportation and Connecting to this Resource.)
Includes utilizing Technology to take advantage of the resources available to us, and navigating the programs provided and in which we will apply for together with our Direct Support.
Workshops and 1on1 Appointments to help people access Housing Voucher Resources for people experiencing Homelessness. As well as people with Disabilities. And people with lower than average income levels - to no-income.
Workshops and 1on1 Appointments to help people navigate and learn how to utilize the United Way 211 phone and online Resource Database, follow ups, and Connecting to the Resources shared with us together as a Team.
Workshops and 1on1 Appointments to help all people Navigate the PEAK Portal to manage and apply for Government Benefits administered at County Levels of Government. (Department of Human or Social Services).
