You will be able to download each file from your own computer so that they may be printed and filled out to and can be dropped off at center, emailed , or regular mailed to us.

To Print on a PC:
Hover cursor over the image of the PDF file and click on the arrow icon you will see in the top right corner of the PDF.
Clicking on the arrow icon will open a new internet tab with the PDF file via Google Drive, in the top right hand corner of that screen you will see a printer icon, which you will then click on.
The printer icon will open another tab, where you will be able to fill in the PDF either on the computer by typing in information and clicking boxes to "check" them, or you can simply print it and write the answers in.
To print, press the "Control" key and the "P" key simultaneously. This will bring you to the printing screen where you are able to select a printer and edit printing preferences, then select the blue "Print" button.

This form is for mileage reimbursement for medical trips only. Once filled out please return the form to the Franklin Senior Center, where it will be reviewed then forwarded to ECTC. Please read this form carefully, as you are releasing ECTC and the towns of Franklin and surrounding areas from being liable if anything were to occur during voluntary transportation.
