STEP 1:Β Download the Citizen's Charter Service Request Form (CCSRF):
Step 2: Once the CCSRF has been duly accomplished,Β
please email it with the documentary requirements toΒ π§ dilgbacoor2023@gmail.com, or hand carry the documents to π¬ DILG City of Bacoor Field Office, 2F National Agency Center Building, Bacoor Government Center Compound, Bacoor Blvd., Barangay Bayanan, City of Bacoor, Cavite 4102 to review the documents submitted.
Once submitted, a notification will be received by the client through email/text/call acknowledging the receipt and to provide other updates on the Certification.
