Intructions:
Submission form Instructions:
Question 1: Is for the name of Barangay Submission.
Question 2: Is the number of weeks totaling the clean-up drive made by the barangay.
You could see what week we are currently at on top of this page. (This page will be updated weekly to show what's the current week number we are at on the clean-up drive)
Question 3: Is a yes or no question, select 1 - if the barangay conducted the clean up drive. And select 2 - if the barangay did not / could'nt because of a weather conditions
Question 4: is the Land area covered. you could see this on the Annex A first table.
if there is no land area but has a water body place a "N/A" on this question
And if the barangay didn't conduct a clean up drive also put "N/A"
Question 5: is the Water Body / Waterway Covered. you could see this on the Annex A first table.
if there is no waterway covered but has a land area place a "N/A" on this question
And if the barangay didn't conduct a clean up drive also put "N/A"
Question 6: is the number of places the barangay cleaned
example is in the next picture
you will count the total area clean from land area covered and waterway covered
And if the barangay didn't conduct a clean up drive put a "1". **We will just edit it out to "0"
Question 7: is the number of attendees participated in the clean-up
you could see them on the attendance sheet
And if the barangay didn't conduct a clean up drive put a "0".
Question 8: is the number of LGU attendees participated in the clean-up
(We were advised that at least one LGU personnel must be on the clean-up drive)
for this one just count the number of LGU personnel on the attendance sheet (if person is appointed/elected in the barangay then they are counted as LGU personnel)
And if the barangay didn't conduct a clean up drive put a "1". **We will just edit it out to "0"
Question 9: is the total number of SK Participated. you could see this on row 3 of the New Attendance format Summary table.

Question 10: is the total number of CSOs Participated. you could see this on row 4 of the New Attendance format Summary table.
Note: CSO count is based on the number of UNIQUE ORGANIZATIONS, not the number of members

Question 11: is the Length Covered. you could see this on the Annex A first table.

Question 12: is the Volume of Waste Collected. you could see this on the Annex A first table.

Question 13: is the Details of Disposal. you could see this on the Annex A second table.
If you want to put space between the two paragraph on the form, you could use the Enter key to get the result the yellow arrow is pointing at

Question 14: is the Other Remarks of the clean-up. you could see this on the Annex A second table.

Question 15: is the Uploading the Clean-up Drive Documents.
The form currently only accept 1 pdf file, so please be advice to merge all the files into 1 pdf file before uploading here.
The sample picture at the top of the questions only shows what pages are high priority on the report submitted on the pdf file

You could also just send the whole report as a secondary option, if the first option is too time consuming
