亞東紀念醫院神經科專科醫師訓練

訓練宗旨與目標

1、簡介

  1. 亞東紀念醫院是醫學中心層級的教學醫院、設有神經醫學部及其他醫療專科合計64科，總病床數1,383床，每日門診服務量超過6,500人次、每日急診服務量近400人次。本院神經科為神經科專科醫師訓練中心、內設有腦中風中心、失智症中心及神經重症中心。為使各級住院醫師獲取神經病學之基本學識與技能及了解神經系統之各種疾病與檢查、俾能達成正確之診斷與適當治療。除門診、病房及加護病房外，另有神經生理檢查區，提供腦波、誘發電位、神經傳導、肌電圖、顱血管超音波及神經心理等檢查，俾能達成正確之診斷與適當治療之需。

2、訓練宗旨

培養可以獨立執行醫療，具神經科學專科知識及技能與自我成長的神經科專科醫師，以提升台灣之神經科醫療水準。

3、訓練目標

訓練完成之神經科專科醫師在面對病人時具備專業素養，全人醫療及以病人為中心的理念與態度。

3.1學習內科病人之基本照護。

3.2培養神經內科疾病之鑑別診斷與正確診斷能力。

3.3核心課程訓練安排培養六大核心能力

訓練計畫執行架構

1、本院神經科專科醫師訓練計畫具明確的執行架構、溝通機制與成效，且包括核心課程、師資、教學資源、訓練課程與訓練方式、考評機制、執行及檢討改進。

2、相關人員包括主持人、教師及住院醫師能清楚知道訓練宗旨與目標，認同訓練計畫的精神。

3、本院院級單位如醫研部能指導與輔助神經內科專科訓練計畫之執行，並每月審視學生學習手冊，適時檢討改進。

4、本院教學部設置教學室、教材室、圖書館、醫事人員教育中心、實證醫學中心、教師培育中心、臨床技能中心，規劃有各種評核制度及表單，如住診教學記錄單、教學記錄單、導師生座談會記錄表、核心課程上課評估表、教學門診記錄單、Healthcare Matrix資料格式、住院醫師年度考核表、住院醫師六大核心能力考核表、及住院醫師考核施行細節。

5、本院醫研部設置學術研發室、共同研究室及動物實驗室，並制定教學計劃，對住院醫師進行醫學研究的訓練，培養其基本研究能力。

臨床訓練環境

1、本院有固定神經科病房 (含NCU)，每年住院病人次約1650人次。

2、本院有獨立之神經醫學部討論室、辦公室與專屬值班室。討論室具備單槍投影、電腦設備與網際網路。

3、本院設置符合醫學中心標準。空間充裕、動線規劃得宜、環境整潔,醫院定時派人打掃。

4、門診(含門診教學)診間皆有明顯告示，門口皆掛有教學門診告示牌，提示病患相關權益說明。教學門診另有專屬教學門診診間，坐落於一樓及二樓門診區，診間內備有單面鏡、高解析度360度旋轉鏡頭及視聽影音設備，可協助教師於另一間診間觀看學員看診情形，以便進行教學回饋。

5、急診訓練：本院提供全天候急診且本院擁有良好急診訓練場所，急診醫學部具有良好的教學醫療環境，區隔出急診檢傷區、急診內科區、急救區、留觀區、外傷區、急診兒科區及特別診療室，以利臨床教師與學生進行臨床教學，急診區設有會議室，提供教學討論的空間。同時，急診訓練場所設有教學專用「教學診療室」與「特別診療室」兼顧病人隱私與學習便利性。

6、病房訓練：各病房設有討論室，提供教師與住院醫師、實習醫學生、進行教學及討論的空間，各病房區皆有網路與電腦設備可連結至醫療系統與PACS系統及圖書館電子資源，對教學及醫療品質上皆具可近性，臨床學習的便利性上也能兼顧。有獨立之神經科NICU (≧5 床)。

7、住診教學訓練：每位住院醫師會加入一個團隊，團隊照護的成員包括主治醫師、住院醫師、專科護理師，以及見實習醫學生。住院醫師會實際負責病人的病史詢問、神經學評估、鑑別診斷，並與主治醫師討論安排檢查及治療的討論，實際觀察病人的治療結果。本院設有住診教學作業管理程序，依其程序執行病房訓練。神經科之病房訓練主要位於11G獨立神經科病房， 有神經科討論室、辦公室與專屬值班室。討論室具備單槍投影、電腦設備與網際網路。

亞東醫院畢業後綜合臨床醫學醫師 (PGY)神經內科訓練

1、訓練目標

1、使PGY醫師獲取神經病學之基本學識與技能，以了解神經系統之各種疾病與檢查，俾能達成正確之診斷與適當治療。

2、神經內科除門診及病房外，另有神經電子檢查室，提供腦波，誘發電位，神經傳導，肌電圖等檢查，以配合疾病診斷之需。各單位均可配合值行相關教學計畫。

2、訓練內容

(1) 加強神經醫學訓練

  1. 病史詢問、理學檢查、鑑別診斷之技巧：

i、 神經病學之基本概念。

ii、病史詢問及病歷記錄。

2.與病患、家屬及對醫療團成員之溝通技巧及人際關係：(利用病房病患照護，學習能與病人/家屬及其他工作人員清楚合宜地溝通，並能用病人聽得懂的話耐心解釋、釐清問題及表現同理心。。

3.落實醫學倫理訓練及實證醫學訓練於臨床照護：醫學資料之查詢。

4.瞭解醫療法規：(透過臨床照護，能遵循現行院內感染管制規則、病人安全目標、醫療品質及健保給付等相關規範。

5. 學習分析醫療品質之良窳及改進之道：參加規定之各種教學活動與討論會。

(2) 病人照顧

1.神經內科重要疾病/主訴之綜合處理(核心能力): 腦中風，失智症，週邊神經病變，巴金森氏症，急性血栓溶解劑治療簡介，意識障礙，頭痛，暈眩，癲癇等。

(3) 臨床技能

  1. 神經系統檢查之方法。

  2. 學習及協助各種特別檢查，如腰椎穿刺、腦電波、神經傳導速度、肌電波、腦血管攝影、電腦斷層攝影與磁振造影等。

3、訓練方式

教學計畫以培養PGY醫師神經內科基本臨床技能及全人照護理念為主，並訂有核心能力要求。配合全院性教學活動，有一般醫學課程訓練，包括全人醫療、病人安全、醫療品質、醫病溝通、醫學倫理與法律、感染管制、實證醫學及病歷寫作等。實際操作則包含會診及住診教學（含床邊教學）、病例研討、醫學影像檢驗等。並於課程結束時給予PGY醫師考核。(實際操作將視臨床情況彈性調整)。另每個月提供4次住診教學及4次核心課程訓練。
