This Patient Privacy Policy outlines our practices and protocols concerning the gathering, utilization, and divulgence of your information within Evidens (referred to as the Service or the App). It aims to inform you about your privacy rights and elucidate how the law safeguards your information.
When submitting images through the App, you are required to eliminate all personally identifying information (or any information that could be used to identify an individual) related to the person depicted in the image, as well as their relatives, employers, or household members. This encompasses, but is not limited to, the following:
Names
All geographical subdivisions smaller than a State or Province in Spain, including street address, city, county, precinct, postal code, and their equivalent geocodes, except for the initial three digits of a postal code if, according to the current publicly available data from the National Institute of Statistics:
The geographic unit formed by combining all postal codes with the same three initial digits contains more than 20,000 people; and
The initial three digits of a postal code for all such geographic units containing 20,000 or fewer people are changed to 000.
All elements (except year) for dates directly related to an individual in Spain, including birth date, admission date, patient encounter date, discharge date, date of death, and exact age if over 89
Phone numbers, fax numbers, personal electronic mail addresses
Spanish social security numbers, national identity numbers, or equivalents
Medical record numbers
Health card numbers, health plan beneficiary numbers, or Health Insurance Account numbers
Account numbers
Certificate/license numbers
Vehicle identifiers and serial numbers, including license plate numbers
Device identifiers and serial numbers
Web Universal Resource Locators (URLs)
Internet Protocol (IP) address numbers
Biometric identifiers, including finger and voice prints
Full-face photographic images and any comparable images
Photographic images of scars, tattoos, piercings, etc., which would allow for identification
Any other unique identifying number, characteristic, or code (note this does not mean the unique code assigned by the investigator to code the data).
The Registered User acknowledges and affirms that they have eliminated all identifiable information. Furthermore, the Registered User asserts that they are not aware that the Images, either independently or when combined with other data, can be utilized to identify an individual who is the subject of the information.
