We’ve condensed all the lineup exchange forms into one!  You select the league in the form and the correct information is populated for you to submit.  This form isn’t mandatory and most captains exchange information for make-ups directly but it’s available for captains who want to use it.  If you are using this form submit it the day the match is cancelled and contact the opposing captain to do the same.
