- You wish to make, change or cancel an appointment
- You want to order a prescription or referral letter
- You have not received a reply to your previous email
We do not accept any identifiable information via email without the consent of the patient. We do not take any responsibility in the event of that information being intercepted.

Identifiable, clinical information will only be sent via email if Dr Jain and the patient has given consent. All emails are sent with an email cover sheet. Patients are always advised on the risks associated when sending clinical information via email such as communications being forwarded, intercepted, printed and stored by others.
Our practice uses the following email disclaimer notice on all outgoing emails and cover sheets:
“This email and any attachments may contain information which is confidential, privileged or subject to copyright. If you are not the intended recipient, please advise the sender by return email and then delete both the original email and any copy of the return email, as you are not authorised to use the email or any attachments.”
Staff are trained at induction on the appropriate use of the practice email. Staff are advised that the practice manager or Doctor have the right check an individuals practice email account if inappropriate use such as fraud, workplace harassment or breaches of confidence of the email is suspected.
