Athletic Clearance

All students, incoming freshmen and returning athletes, who wish to participate in any school sport, at El Modena High School, must have Athletic Clearance on file, with the schools health office and athletics department.

Athletic Clearance must be completed every year prior to participating in any athletic activities or trying out for any sport, including Girls Basketball.

To obtain Athletic Clearance you will need to

• EMHS Physical Examination Form (Physician fills out)

• Health History Form (Parents and Athlete fill out)
