TRANSCRIPT REQUESTS

Verification of Graduation:
Agencies requesting proof of graduation for pending employment must fax the signed release form for the applicant to (619) 397- 3920.
Eastlake High School Students Official Transcript Request:
Please email the school registrar responsible for your alpha with your request.
Fee: Free
Please include your first name, last name and student ID number
Please provide mailing address information (Office of Admission) for official transcripts requested by mail.
For students applying to Universities through the Common App the counselor will submit your transcript on your behalf.
Eastlake High School Graduates:
Please email the school registrar responsible for your alpha with your request.
Fee: $2.00
Provide the following information:
Year of graduation and name used while attending high school
Birth date
Copy of driver license
Address where you want transcript sent
Fee: $2.00 per transcript

WORK PERMITS
Students who attend Sweetwater Union High School District are required to obtain a work permit if they are under 18 years old, and have secured paid employment. The work permit is required by CA Education Code and is enforced by the CA State Labor Department. Sweetwater Union High School District students must meet and maintain 2.0 G.P.A. (grade point average) and attend school regularly and on time to obtain and keep a work permit.
Work permits are processed at the Eastlake High School Counseling Center by Ms. Niemeyer, the Counseling Secretary. If you are interested in obtaining a work permit please contact Ms. Niemeyer by email at Betty.niemeyer@sweetwaterschools.org. Please include your name and student ID number. Your application may take up to 5 school days to be generated. Your attendance and academic grades need to meet the requirements in order to issue the actual permit. You will be informed by email once it is completed.
ENTERTAINMENT BUSINESS
Students who are in the entertainment business need to obtain the permit from the:

Students will need to obtain, from their home site, their grades, (students cannot have any D’s or F’s). attendance and official school seal. The school site will not be giving out any work permits dealing with the entertainment business.
An Assistant Principal must fill out and sign off on these forms. The Registrar will then stamp it with the school seal.
