Reimbursements

Do we owe you money? If so, thanks for spotting us your cash and we'd be happy to reimburse you if you do the following:

1. Download this form as a PDF or make a copy of this google doc

2. Fill it out, usually using Adobe Acrobat Reader

3. Take pictures, export, or scan supporting receipts 

4. Email the PDF and receipts to treasurer@dwpta.org with the subject line "Reimbursement Request"
