To refer someone to the service, please complete the Gateway Form provided by Dudley Council (click link below). Please provide as much information as possible and enter all requested information in boxes. Once completed, please send to: supportingpeople.dachs@dudley.gov.uk. 

Please note, you will need to download Adobe Acrobat to open the PDF referral form. You can download it from your browser (CLICK HERE) or the app / Microsoft store. Once downloaded, go onto your documents/downloads on your desktop, right click, open the PDF using Adobe Acrobat from where you will be able to edit. If it will not let you download, please speak to your IT department.
