In some cases, you may not need all the options listed below. Please review and choose the best fit for your situation.
Option A:
Complete the Reasonable Accommodation intake form
Email your current medical records for review
Receive a medical records review and a 15-minute consultation
Fee: $75
Option B:
Complete the intake form
Email your medical records for review
Receive my CV, a one-page report with your reasonable accommodation assessment, and peer-reviewed research support
Fee: $250
Option C:
Complete the intake form
Email your medical records for review
25-minute consultation
Receive my CV, a detailed 1-2 page report, including agency RA forms if applicable, and peer-reviewed research support
Fee: $300
Once you choose your option:
Email your medical records and submit payment. Once proof of payment is received. You will receive a confirmation email with follow-up instructions based on your selected option.
Most reports are completed within 3 business days.
Need it faster?
An expedited service is available for an additional $75, with a 24-hour turnaround.
