• I will analyze everything and provide a 25-minute consultation

• Receive a detailed report, including agency-specific OPM forms, peer-reviewed research, my CV, and the completed OPM SF-3112 form

Option D: Premium Comprehensive Support (Top Tier)

Fee: $1,500

Includes everything in Tier C, plus:

1. Medical Sick Letter

2. Reasonable Accommodation Letter

3. Strategic integration of all Tier C documentation and benefits

4. Enhanced, fully tailored reporting designed for complex or high‑stakes cases

5. Priority review and support

6. One follow-up letter for attorney 

7. Medical psychological diagnosis (not guaranteed), if clinically appropriate, based on comprehensive clinical assessments, structured interviews, psychological testing, and thorough medical records review

Additional Premium Benefits:

1. One-time letter adjustment or follow-up, if requested by an attorney, or a one-time revision to the letter for the client

2. One-time 15–20-minute phone conference call with an attorney, if needed

4. One mailed hard-copy letter sent to either the attorney or the client

• An expedited service is available for an additional $250, with a 24-hour turnaround.

• Your report will be emailed to you within 3 business days after all steps are completed.
