Transition planning and navigating life after high school can be a daunting process for students and their families- Door CCoT is here to help. With so many options and opportunities available, you have the ability to create a life that fits your vision. Sometimes the most difficult part is figuring out where to start and visiting our resource page is a great step! A good place to start is reviewing the Transition Planning Tools page. Check out the menu for different areas of interest which will give you information and options available in Door County. You can also use the search eyeglass in the upper right hand corner to search for specific information.

We are a resource to people with disabilities, their families, teachers, and stakeholders to:

The Door CCoT group meets the third Tuesday of every other month from 12:00 pm to 1:00 pm.
Schools, agencies, parents, students, and community members are welcome and encouraged to join us!
