Bay Lake Consortium

Door County is part of the Bay Lake Consortium. The Bay Lake Consortium is a group of county agencies that manages Medicaid, Medicare Savings Programs, and other benefit programs. You can call the Bay Lake Consortium at 1-888-794-5747 if you want to apply for those programs, report changes, or ask questions. You can fax verification documents to Wisconsin’s Centralized Document Processing Unit (CDPU) at 1-855-293-1822.

Medicare

If you have questions on Medicare, you can call 1-800-633-4227. You can also visit the Medicare website at http://www.medicare.gov/https://www.mymedicare.gov/ 

Social Security

It is important to have a Social Security number. A Social Security number is needed to get a job, collect Social Security benefits and get some other government services.  Social Security provides financial assistance to people who are unable to work due to a disability. Applications for adult disability benefits can be filed the month the applicant turns 18. Complete a disability application online at: https://www.ssa.gov/benefits/disability/

You can contact Social Security to report changes or if you have questions on benefits, the Medicare Part D Extra Help, etc. The phone number for the Green Bay Social Security office is 1-888-862-4811. You can also call the national toll free number at 1-800-772-1213 or visit Social Security’s website at http://www.ssa.gov/.

If you are already connected with disability benefits and want more information on how employment will impact those benefits, please complete a referral to the Division of Vocational Rehabilitation (DVR): 

A work incentive benefits analysis is intended to support a person with a disability in understanding the relationship between their employment goals and their benefits. These services should be provided throughout the VR process but are especially important when changes to employment or benefits occur. It is generally a point-in-time analysis, and additional information and/or analysis over time may be needed.
