Program Ads

For your convenience, three options are available when you place your ad. 

Option 1 - Complete the online form, upload your artwork, and pay online. A convenience fee of 4.0% will be added to your invoice for payment by debit or credit card. For example, if your total invoice is $150, the convenience fee will be $6.00 for the total amount due of $156.00.  

Once you have completed the online form, you will immediately receive an email with a link to make the required payment.

Option 2 - Complete the online form, upload your artwork, and mail a check.

Option 3 - Print the pdf form, mail the completed form with your artwork and check for payment.

PAYMENTS AND ARTWORK MUST BE RECEIVED NO LATER THAN
MARCH 1, IN ORDER TO PREPARE THE PROGRAM.

• Two examples are provided for the Tribute ads.

• If you choose a tribute ad, FVDG Roy will contact you to ask for the information for your ad.
