In an effort to manage hospital capacity, the division has identified opportunities to transfer patients to the USNS Comfort and the Javits Center. Identification of appropriate transfer candidates should be done by the supervising hospitalists.

Recommended talking points for patients identified as appropriate for transfer.
Case Finding – parallel processes
a. Case Management: patients identified during SWAT rounds to “CM Discharge Commander”
b. DHM: Supervising Hospitalists identify and notify the Consult/Lead Hospitalist
Consult/Lead Hospitalist contacts treating team (usually the attending-of-record, though can be any team member)
Team informs patient/family that they may be transferred to a low-acuity site. Note: The patent may not yet be officially accepted by Javits/Comfort.
a. Patients/families are not asked; they simply need to give assent (assent is assumed given if they do not state they refuse to be transferred). Team should use suggested scripted language. [LINK]
If patient, agrees, notify your Supervising Hospitalist.
The transfer process (including communicating with the receiving facility and arranging for transportation) is completed by the MSHS Transfer Center.
You will receive notification from the Transfer Center, Lead Hospitalist, or your Supervising Hospitalist when the patient has been accepted and transportation has been arranged.
Once you receive confirmation:
a. Notify the patient and family
b. Complete the discharge summary
c. Complete the medication reconciliation and e-Prescribe a five day supply of all medications to the Icahn School of Medicine Pharmacy. The medications will be delivered to the patient's bedside.
d. Follow up appointments and quarantine instructions will be completed by the receiving facility.
