The purpose of these rounds is to briefly discuss patients that are active or those that may be improving and nearing discharge. The social worker and case manager on your unit will help identify barriers to discharge. These rounds are also an opportunity for you to identify new needs that may need to be arranged (such as home oxygen, home physical therapy evaluation, etc.).
The department now uses the "Discharge Today" tool to help improve communication amongst the interdisciplinary team. Add the "Discharge Today" column to your patient list to access this tool. Clicking on the column will allow you to designate whether someone is medically active or nearing discharge. This tool should be completed daily.
