EVIR - Employee Violent Incident Report
According to the Ontario Health and Safety Act (OHSA)
“Workplace violence” means:
the exercise of physical force by a person against a worker, in a workplace, that causes or could cause physical injury to the worker
an attempt to exercise physical force against a worker, in a workplace, that could cause physical injury to the worker
a statement or behaviour that it is reasonable for a worker to interpret as a threat to exercise physical force against the worker, in a workplace, that could cause physical injury to the worker
SSIR - Safe Schools Incident Report
OCDSB employees are required to file an SSIR when they become aware of a student behaviour that could lead to suspension or expulsion, according to provincial guidelines.
Complete this for form if you have any health and safety concern that you would like to be addressed by the Joint Occupational Health and Safety Committee
If you have an accident or a near miss while at work or on the employers property you are required to report it. Speak to your supervisor to fill out the online accident report together.
If you are injured and have lost time at work you will also need to report it to the Workplace Safety and Insurance Board (WSIB)
