CONSENT FORM

IN RESPECT OF EU GENERAL DATA PROTECTION REGULATION 2018 AND THE UK DATA PROTECTION ACT 2018

I UNDERSTAND:-

a)      that my consent may be withdrawn at any time by notice in writing to the Club Secretary (by post or email) whereupon my details will be removed from Club records within 28 days (Membership Directory excepted).

b)      that on ceasing to be a Member of the Club I will destroy all records relevant to my former Membership

c)      that this CONSENT FORM shall be valid for the total period of my Membership unless I have advised the Committee to the contrary
