Must be a resident of Orange County, and have no health insurance coverage. Have the patient go here: https://www.gracemedicalhome.org/become-a-patient; Download the Adult Patient (or Child Patient) Intake form in English or Spanish.

Complete the form before calling Grace Medical. On the 27th of each month, have the patient call Registration/Intake for new patients. (Patient must be the one to call. They take first come, first served.)
