The Academy of Oncology Nurse & Patient Navigators (AONN+) is the largest national specialty organization dedicated to improving patient care and quality of life by defining, enhancing, and promoting the role of oncology nurse and patient navigators.

The Alliance promotes policies, programs, and partnerships that: reduce and eliminate barriers to quality health care both within health systems and the community; reduce disparities in health outcomes; and foster ongoing health equity. The Alliance believes that every Coloradan has an equal opportunity to obtain healthcare that personally addresses their medical, mental, emotional, social determinants, and spiritual health needs.

American Cancer Society provides information and resources about prevention, early detection, treatment and survivorship for all cancer types. Resources include education resources for patients and healthcare systems, as well as barrier reduction services for cancer treatment.

The Health Navigator Workforce Development Initiative aims to define competencies and standardize training for unlicensed health navigators as well as create a voluntary registry for employers to use when hiring.

The CCC is a statewide, nonpartisan, multidisciplinary, coalition providing education, networking, best practice sharing, and partnership opportunities for those working in oncology care and support. In addition, we support the needs of Coloradans to prevent cancer, detect cancer early, and enhance quality of life for survivors and their caregivers by disseminating education, cancer resources, and connecting patients and their caregivers to opportunities to advocate for better cancer outcomes in Colorado. 

The Colorado Community Health Network (CCHN) represents Colorado’s 20 Community Health Centers (CHCs) that together are the backbone of the primary health care safety-net in Colorado. 

Regional Health Connectors (RHCs) are a locally embedded workforce that aim to improve health in Colorado by connecting the systems that keep us healthy – including primary care, public health, social services, and other community resources. Serving as liaisons to physician offices, public health and community organizations, RHCs create partnerships that lead to healthier communities and healthier practices across the state. 

The Colorado Rural Health Center (CRHC) is Colorado’s nonprofit State Office of Rural Health. CRHC works with Federal, state and local partners to offer services and resources to rural healthcare providers, facilities and communities. 

Fight Colorectal Cancer (Fight CRC) fights to cure colorectal cancer and serve as relentless champions of hope for all affected by this disease through informed patient support, impactful policy change, and breakthrough research endeavors.

The National Colorectal Cancer Roundtable, established by the American Cancer Society (ACS) and the Centers for Disease Control and Prevention (CDC) in 1997, is a national coalition dedicated to reducing the incidence of and mortality from colorectal cancer in the U.S., through coordinated leadership, strategic planning, and advocacy. The ultimate goal of the NCCRT is to increase the use of proven colorectal cancer screening tests among the entire population for whom screening is appropriate.

The National Lung Cancer Roundtable (NLCRT) is a national coalition of public, private, and voluntary organizations, and invited individuals, dedicated to reducing the incidence of and mortality from lung cancer in the United States, through coordinated leadership, strategic planning, and advocacy. 

The NNRT serves as a catalyst to initiate work on key issues around patient navigation, as well as to disseminate best practices and to overall enhance the field of navigation. A fundamental premise of the Roundtable is that collective action among member organizations will be more successful in advancing the field, than if they worked alone. Together, members work to share information, identify needs and opportunities, while addressing gaps relating to navigation.

The Patient Navigator Training Collaborative is based in Denver, CO and provides national leadership for the development, education, standardization and sustainability of the growing patient navigation workforce.

Women’s Wellness Connection (WWC) offers free breast and cervical cancer screenings at more than 100 clinics across Colorado to those who qualify. We also provide health navigators who can help you with the process of getting the right tests, including mammograms and Pap or HPV tests, and any needed follow-up tests.
