How to Fill Out the Stand Booking Request Form
The Stand Booking Request Form consists of three pages and five sections. Please note that fields marked in red are mandatory. If not filled out, we will be unable to process your application.
I. EXHIBITOR GENERAL INFORMATION
Date: Please start by filling in the date in the appropriate top left field in the format DD/MM/YY.
TAX ID Number*: Please fill in your company's Tax Identification Number (for U.S. companies, this can be found on your company's W-9) - If you are not sure what the equivalent of Tax ID is in your country, please click here for more information (please be sure to also provide an English or Spanish translation of your Tax ID document if it is in a language other than English or Spanish).
Company Name: Please fill in the full legal name of your company
Commercial Acronym: Please fill in only if available.
Address / City / Country / Telephone / Fax / Corporate E-mail / Website: Please fill in your company's legal address, phone, and fax (if available).
CIIU Code: No need to fill, this is only applicable to Colombian companies.
E-Billing Section - Email Address / Responsible(s) Electronic Invoicing / Telephone: Please fill in the e-mail, full name, and phone number of the person(s) from your accounting department to whom the invoice needs to be sent.
Legal Representative and Directors Information: Please fill in the passport number, full name, position, cell phone, and E-mail of your company's Legal Representative. This can be the President or CEO of your company or someone with the faculties and authorization to act as Legal Representative of your company when filling out and signing this application and the later contract that will be issued by show management.
In the Director's section below, you may also add the General Manager and/or Sales Manager details as well.
Those Responsible for Participation: Please add here the main contact or contacts that will be manning your booth at the show.
Stand requires overhead sign installation (basic assembly): Tick "YES" if you're acquiring one of our booth packages. Tick "NO" if you're only booking raw space and using your own construction team to build your booth.
If you selected YES, in the blank space below, please write your company's name, written exactly as you would like it to appear on your booth's overhead sign.
Commercial Name of the Company in the Show Catalog: Write your company name exactly as you would like it to appear in the printed show directory.
Stand construction: Tick " Standard shell schemes (available options Gold, Silver, or Bronze)" if you're if you're acquiring one of our booth packages. Tick "Individual stand construction with own stand constructor" if you're only booking raw space and using your own construction team to build your booth.

*Information and proof of business identity is required by Colombian Federal Law. The Colombian government is committed to preventing illegal business practices, such as narcotics trafficking, money laundering, and bribery. By law, this is a standard procedure that MDNA and our co-organizer Corferias must comply with. Please be assured that any propriety and private information and documentation provided is kept strictly confidential. This long-standing procedure is something that all international exhibitors at trade shows in Colombia must comply with in order to conduct business there and is a security measure designed also for the exhibitor's own protection.

V. METHOD OF PAYMENT
Payment for participation is due as listed:
40% Due Upon Admission
30% Due before February 28, 2024
30% Due before July 26, 2024
Note: In accordance with the contract’s Terms and Conditions, cancellation or withdrawal from ColombiaPlast 2024 after admission results in a penalty fee of 30% of the total exhibit space price.
