VERIFICATION OF TRAINING Records 

The following form submission will request a listing of all current certifications in the First Response Health & Safety Database for a student. This list will come by Email and allow up to 48 Hours for results.

You will need the following information to enter:

Students Name,  Students Email Address, Students OPIN #,  Plan Member ID # (PM Code), Requester's Name, Requester's Email, Requester's Phone #
