Have you completed Step 2? The following tool is specific to Town Hall Meeting event types. The modification and download options are applicable to the post- Town Hall Meeting survey tool.

Modification of Paper Version of the Survey Tool

Level of Difficulty: Easy

Estimated Time: 5 minutes

Considerations: Use this if you are unsure of the internet access at the event location. We always recommend having a couple of hard-copies on hand just in case something permits a user from completing the survey online.

What you need to have to accomplish:

  1. Download either the WORD Version or PDF Version of the following tools.

  2. The only edits we recommend are updating the Community Issue at the top of page 2.

Post-Town Hall Meeting Survey

Description: This survey should only be completed by those attending a Town Hall Meeting event. This should not be completed by those attending a Community Forum event (select for Community Forum tools).

Considerations:

  • For convenience to participants it is recommend to distribute a paper version of this survey as participants arrive to the event (asking not to complete until the end of the meeting).

  • After the town hall meeting an administrator (Extension Agent) will upload participant survey responses to the online survey tool using the customized survey link you created as part of step 2.

  • Spend 5-20 minutes to create an event-specific survey tool.

Steps to get generic printable survey tool:

  1. To edit in Word, access the Post- Town Hall Meeting Survey Tool Word Version.

  2. Select File in the tool bar.

  3. Select Download.

  4. Select Microsoft Word (.docx). Typically, you will be asked where you want to save the document and how you want to name it. If this occurs we recommend adding the event date and county name as a pre-fix to the document name (i.e., 06152020ProtectIRL).

  5. Locate the downloaded/ saved file on your hard drive.
