REGISTRATION

1. Visit and fill in the CHW Online Course registration form.
2. Pay fees at TTUHSC West Texas AHEC CHW Program site.
3. West Texas AHEC will send a confirmation email for your registration, payment, and course information.

Fees include complete course and materials.

• Per Participant: $500.00

Special accommodations: Reasonable accommodations for individuals who qualify under the Americans with Disabilities Act are available upon request. Please notify the program instructor as soon as possible. Classes will be offered in English only.

• Payment options: Full $500 or 5 months at $100
• Discounts to employers training multiple employees. Contact CHWBridge@ttuhsc.edu for more information.

Incomplete registration forms will not be processed.

Payment Method:

• Make checks or M.O. payable to West Texas AHEC CHW Certification Program
• Pay by card (Visa or MasterCard)
  • Visit at TTUHSC West Texas AHEC CHW Program site to pay online.

Non-Payment will result in inability to submit application for certification. Students will not receive refunds if they drop out of the program.

• West Texas AHEC is a Registered CHW Apprenticeship Program and will offer one interested student an opportunity to become the first apprentice.
• Information about the apprenticeship program will be discussed with students accepted into the program.
• Applicant must be a Lubbock, TX resident
