Welcome to Our forms center! As a gMAC member, it is importnat that you have all of your forms turned in (Membership Contract, Trip Waiver, Academic Plan, etc). Below are pdf's/links to all forms needed as a gMAC member. If you need a new document, feel free to print direclty from here and and it to a member of Exec Board. 

gMAC Membership Contract form 

All members are required to sign this contract form, showing they understand the expectation that come with being a MAC member. It also shows that they agree to uphold the core values of gMAC. It is important that you read all of the information on the document. 

gMAC Field Trip Waiver Form 

In order to come with us on our various trips around New Jersey (and New York!) We must make sure you, as a member, and your parents are ok with taking different forms of transportation to our events (i.e. NJTransit, School bus, walk, etc.)!
