Positions within the NHS
This document will list all the positions within the NHS from trainee to Medical Director. The job details and promotion requirements.
Promotions will not be given based on the time spent in your position, it will be solely based on your performance in your position. The times are there to give you insight to the earliest you can get promoted in usual circumstances. Although the times are there if you are seen my command constantly giving 100% and seem fit for a promotion it could occur earlier.
Paramedic Positions
Emergency care assistant
[ECA]
In simple terms; this is someone who is yet to complete their paramedic training. This position will be the standard entry point to the service.
Paramedic
[P]
This is the position you will be in after completing your initial paramedic training, you will be free to attend calls on your own.
Specialist Paramedic
[SP]
After being a paramedic for around 2 weeks you may be put in for a promotion to Specialist Paramedic. As a specialist paramedic you will have access to more equipment.
Consultant Paramedic
[CP]
After being a Specialist paramedic for around 6 weeks you could be put in for a promotion to Consultant paramedic. As a consultant paramedic you will be able to help do ride alongs with ECA’s, doing these could help put you forward for earlier promotions and bonuses etc.
Nursing positions
Nursing department is different from paramedics and doctors. As a nurse your main focus will be looking after patients in the wards and A&E. You will run initial assessments to decide whether A&E patients need care immediately or a medical appointment.
Healthcare Assistant
[HA]
A Healthcare Assistant will be the entry position to nursing. In this position you will receive training and shadow a Senior nurse.
Nurse
[NE]
After receiving your nurse training, you will be promoted to nurse. Your main duty will be helping out in wards and A&E. This will be asking any questions, routinely check ups etc. You will be trained in making initial assessments in A&E.
Senior nurse
[SN]
After 3 weeks as a nurse you will potentially be put forward for promotion to Senior nurse. As a senior nurse you will be trained in discharging patients from the wards. You will also help HCA with their shadowing process.
Matron
[MT]
Matrons will be selected by the head of nursing. As a matron you are essentially a team leader for the nurses. You will monitor the nurses, bring issues up to the head of nursing, direct nurses to different sections of the hospital etc. This could be considered ‘Low command’.
Doctor positions
Foundation Doctor
[FD]
Foundation doctors will be a position you can apply for once being a CP for around 2 weeks. This role is application based as you will be required to spend a bit of time around the hospital attempting RP with patients. You will still be able to carry out your paramedic duties.
Doctor
[DR]
After around 3 weeks of being a FD you can be put forward for promotion to DR. As a doctor you will be expected to spend quite a lot of your time in hospital as well as going out onto the streets. As a doctor you will be trained to conduct medical tests for the police department.
Specialist Doctor
[SD]
Once being a DR for around 4 weeks you will be eligible for a potential promotion to Specialist Doctor. As a SD your main role will be hospital based. You will be trained in appointments for the public. Any citizen who has a medical issue that isn’t an immediate issue will be directed to a Specialist Doctor for an appointment. Although you will be hospital based, you will be permitted to carry out paramedic duties too
Command Positions
These positions are positions selected by executives. As a command member you are in charge of either a department, or sub division in the NHS.
Clinical Manager
[CM]
As a clinical manager you will be supporting the Operational manager of your department. This will be supporting trainings, helping with databases etc.
Operational Manager
[OM]
Operational Manager is the highest non-executive position you can achieve. You will be picked out carefully by executives. In this position you are expected to run the department with your Clinical Manager.
