Center for Excellence in Primary Care: Under “tools for transformation” you will find videos and teaching modules for health coaches, case managers and care coordinators on health coaching, medication reconciliation, team care, etc.
California Health Workforce Initiative: Product of the Workforce and Economic Development Division of the California Community Colleges Chancellor’s Office. Curriculum with easily utilized teaching modules on six “soft skills” for medical assistants including team care, motivational interviewing, embracing diversity. Free “train the trainer” sessions have been made available across the state.

Return on Investment Calculator by Victor Tabbush. The calculator facilitates return on investment projections, allowing for adjusting calculations based on staffing levels and variable costs. National Academy of Medicine: Effective Care for High Need Patients. Review of 14 successful models of care for high needs people detailing: (1) key characteristics of high-need patients; (2) the use of a patient categorization scheme—or a taxonomy—as a tool to inform and target care; (3) promising care models and attributes to better serve this patient population, as well as insights on “matching” these models to specific patient groups; and (4) areas of opportunity for policy-level action to support the spread and scale of evidence-based programs.

Institute for Healthcare Improvement (IHI) Framework for Improving Joy in Work. Alameda Health System has adopted IHI's approach to improving joy in work as a strategy to create a work environment that is positive, solution oriented and engaging. The framework involves 1) what Matters to You? conversations with leaders, providers and staff about what’s currently working and what needs improvement, 2) identification of locally managed improvements (pebbles) and system level issues (boulders), 3) development of an action plan and use of QI to drive improvement, 4) communicating progress to all stakeholders. - Holly Garcia, Cohort 13
