Complex Care
This group is a collective of fellows and alumni of the CHCF Health Care Leadership Program with interest in and experience with patients with complex health and social needs. They join together to share innovative practices and resources, learn about practices others use in advancing work in the topic area, and serve as content experts.

Innovation Group
The CHCF Alumni Innovation Group was created to bring together a community of catalytic individuals to share their leadership journeys in bringing innovative models in their healthcare system.
