EPA Mapping
This simplified EPA mapping guide is intended to help you plan your EPA requests on your core rotations. Some EPAs may be listed (and thus are able to be requested) in multiple categories/rotations. However, special note is made when a specific EPA can only be be done in a certain location (i.e. only possible on an off-service rotation, such as ICU).
*Note that this guide is not intended to be a complete representation of the EPAs you may request on a rotation, but is meant to provide you with a starting point.

While Elentra is the default EPA system at Western, electronic fillable PDFs or printed PDFs of the relevant EPA assessment forms are linked below for any services or elective blocks that may not have access to Elentra.
PDFs of assessment forms
Print/send out a copy of the EPA assessment form to give to your Observer/Assessor
Request the EPA and explain what it is
Ensure you receive the completed form—don't forget to ask for feedback before the assessment takes place so that the feedback you receive is constructive and based on an observation!
Upload your completed form in ePortfolio no later than 7 days after your request
