Before administering the printable version of the CCBS, please review the manual. This one-page document will assist you (or any other supervisory/administrative staff) with the logistics of administering the CCBS and help you interpret scores.

This document contains the actual questionnaire for staff (the CCBS) ready to print. This attachment is in a landscape orientation to accommodate two copies of the CCBS per page. After printing, please cut each sheet of paper in half to yield two questionnaires.
In order to save paper, this option is recommended.
