Digital Version

The digital version of the CCBS is available on website called JotForm.

1. First, go to https://www.jotform.com and create an account.

2. Next, locate the CCBS template by following this link: https://www.jotform.com/form-templates/ur/camp-counselor-burnout-scale-ccbs and click “Use Template” to save the CCBS to your account.

3. At this point you will be brought to a page that says “Build” where you are given the ability to modify the questionnaire. Please do not make any changes. Instead, click “Publish” at the top of your browser window.

You will see a link that you can now share with your staff. This link will direct them to the version of the CCBS that is tied to your account, meaning that you will be able to see the responses on your JotForm account but nobody else, including the developers of this scale, will be able to access your data.
