Make sure to bring a copy of the forms to our play. We will know it’s a copy because the originals are written in blue in and the copies are in black ink. You can fill out the form online but the date must be in blue ink.
Kids not doing project boards
- You can show the board through a power point.
- Students will have to print the power point first.
- There will be NO ELECTRICITY.

Let’s review together: We highlighted for you. Pink is before experimentation and yellow is after.
There are forms that are required:
- Check list for the adult sponsor: Usually the science teacher.
- Check list for the student: The experimental dates, look at line 7 there is a start and end dates. Make sure these dates are correct. Theses’ are the dates they started to collect data. THEY MUST ALINE WITH WHAT IS IN THE LOG BOOK.
- Research plan: There is a template on our website to use to the research plan. Make sure that the dates are BEFORE the experimentation. For continuations, they would add to an existing plan.
- Approval form B: This form causes frequent disqualifications of projects. Make sure that the dates are BEFORE they start of the experiment. Do not forget to fill out the bottom (section 2) of this form. Remember to get the approval for BHBAs (bio hazard) from the SRC. If your school does not have and SRC we will sign off for you. The bottom box (section 3) is for Dr. Johnson as the district SRC.
There are forms that SOME projects must have.
- Regulated Research Instructional Setting Form (2 Pages): Dates must be after FORM 1(a) line 7 (experiential end date). Make sure to answer all questions. Second page has the signature box.
- Qualified Scientist Form (2)- Make sure that the person has advance knowledge of the subject area. This form is filled out and signed by the qualified scientist. This form is BEFORE experimentation. Make sure to look at question 4,
§ if the answer is no fill out both boxed.
§ If the answer is yes then the supervisor will fill out the left box.
- Risk assessment (3): Filled out by qualified scientist or designated supervisor. Use this form for “special” projects. ie: microorganisms, vertebrates, power tools, heater…. This form is to help minimize the risk and keep each student as safe as possible.
- Human Participants Form (4): If there were human test subjects they must do this form. Top is filled by the student and the bottom is filled by your schools IRB. There is no district level IRB. Remember all three members of your IRB should sign and date this form BEFORE your experiment begins.
- Human Informed Consent Form: This goes in conjunction of line 6 of Human Participants Form. For every test subject in the study. This has to be done BEFORE they start the experiment.
- Verification of Informed Consent: This is required to go with the Human Participants Form and The Human Informed Consent form. This form is filled out by the adult sponsor. DO NOT FORGET THE REFERENCE IN THE BIBLIOGRAPHY
- Vertebrate Animal Form (5A): You must have a mortality form and a reference how you took care of the animals. NO animal can die as a result of your experiment.
- Vertebrate Animal Form (5B): Only if the research was done at a research facility. Make sure to attach the Protocol number of the IACUC.
- Potentially Hazardous Biological Agent Risk Assessment Form (6A): Not recommended for middle school. Must also do a BSL risk assessment (BSL2 disqualifies middle school). Must reference microbiological technique in bibliography. These are signed BEFORE the start of data collection. Section 4 will be filled out by Dr Johnson.
- Biosafety 1 and 2: (BSL1 and BSL2) These are just the checklists for working with microorganisms.
- Human and Vertebrate Animal Tissue Form (6B): Must be done with a 6A. Must be done BEFORE experiment
- Continuation/Research Progression Projects Form (7): Must attach the research plan and abstract from the previous year. Must be significantly different from the prior project.
- Abstract: The abstract form for INTERNATIONALS is different from the STATE form. There should be 13 categories. Check the https://ssefflorida.com/categories/ to make sure you chose the correct category.
Q and A:
- Log book should be dated.
- Keep MSDS sheets in the log book and reference it in the bibliography.
- make sure to add cleaning agents to the risk assessment and MSDS area.
- Date on Abstract is AFTER, remember its a summary.
- Remember no QR codes, name brands, or faces on boards.
- Do not forget to site all pictures on the board.
