They rate the Severity (S) of the effect, the likelihood of Occurrence (O) of the cause, and the likelihood of Detection (D) using existing controls (typically on a 1-10 scale).
Risk Priority Number (RPN) is calculated (S x O x D). Higher RPNs indicate higher risk and are prioritized for action.

Choose Unit of Measurement: Decide what you will measure for each category (e.g., frequency/count, cost, time lost). Frequency is most common.
Define Time Period: Specify the timeframe over which the data will be collected (e.g., one week, one month, one quarter).
Collect Data: Gather data for each category over the defined period. A Check Sheet is often a useful tool for this step.
Summarize Data: Tally the counts (or sum the costs/time) for each category. Calculate the grand total for all categories.
Rank Categories: Order the categories from the largest count (or cost/time) to the smallest. Place any "Other" category last, regardless of its count.
Calculate Percentages:
Individual Percentage: For each category, calculate its percentage of the grand total: (Category Count / Grand Total) * 100%.
Cumulative Percentage: Calculate the cumulative percentage for each category by adding its individual percentage to the cumulative percentage of the preceding category. The first category's cumulative percentage is its individual percentage. The last category's cumulative percentage should be 100%.

The "Head" (Effect/Problem): This is located on the right side of the diagram, typically enclosed in a box. It contains a clear, concise statement of the specific problem or effect being analyzed. Crucially, the problem statement must be well-defined and agreed upon by the team before starting. (e.g., "High rate of medication errors," "Low customer satisfaction scores," "Excessive machine downtime").
The "Spine" (Main Arrow): A horizontal arrow pointing towards the head (the effect). This represents the main line connecting the causes to the effect.
The "Main Bones" (Major Cause Categories): These are diagonal lines branching off the spine. Each main bone represents a primary category of potential causes. The categories chosen should be relevant to the problem being analyzed.
The "Smaller Bones" (Specific Potential Causes): These branch off the main bones. Each smaller bone represents a specific potential cause identified during brainstorming that falls under that major category.
"Sub-Causes" (Deeper Causes): Smaller bones can have further branches representing causes of causes. This allows for drilling down towards more fundamental root causes, often revealed by asking "Why?" about a specific potential cause listed.
5. Common Major Cause Categories:
While categories can be customized, several standard sets are widely used:
The 6 Ms (Common in Manufacturing):
Manpower (or People): Factors related to human resources, skills, training, motivation, experience, etc. (e.g., inadequate training, lack of attention, insufficient staffing).
Method: Factors related to processes, procedures, work instructions, standards, etc. (e.g., unclear procedure, incorrect sequence, poor process design).
Machine (or Equipment): Factors related to tools, machinery, equipment, technology used, maintenance, etc. (e.g., machine malfunction, tool wear, outdated software).
Material: Factors related to raw materials, components, supplies, consumables, information, etc. (e.g., defective raw material, incorrect specifications, poor quality data).
Measurement: Factors related to data collection, inspection methods, gauges, calibration, definitions, etc. (e.g., inaccurate gauge, inconsistent measurement technique, unclear defect definition).
Mother Nature (or Environment): Factors related to the physical or operational environment, conditions, regulations, culture, etc. (e.g., temperature fluctuations, high humidity, poor lighting, time pressure).
The 4 Ps (Common in Service/Marketing):
Policies: High-level rules guiding decisions.
Procedures: Specific steps for carrying out policies.
People: Human factors (as in 6Ms).
Plant/Technology: Equipment, facilities, IT systems.
