Age Requirement:

• Kindergarten 1: 4 years old

• Kindergarten 2: at least 5 years old by August 31

Elementary & Junior High School

For New / Incoming Students:

• PSA Birth Certificate (photocopy)

• Report Card (SF9), Elementary Report Card (SF9) for Incoming Grade 7

• Certificate of Completion (for Grade 1 & Grade 7, if applicable)

• Recent 2×2 picture

• Accomplished Enrollment Form

For Transferees:

• Report Card (SF9)

• Good Moral Certificate (for transferees)

• Certificate of Enrollment / Transfer Credentials (SF10 

Senior High School (Grades 11–12)

Required Documents:

• PSA Birth Certificate (photocopy)

• Junior High School Report Card (SF9)

• Junior High School Certificate of Completion

• Good Moral Certificate

• Recent 2×2 or passport-size photo

• Accomplished Enrollment Form

• Track / Strand Application Form (if applicable)

For Transferees:

• Latest Report Card (SF9)

• Transfer Credentials (SF10)

• Good Moral Certificate

Step 1: Inquiry and Information

Parents/guardians may inquire at the School Registrar’s Office, through the school website, or via the school’s official communication channels. Information regarding grade levels, programs offered, requirements, and fees will be provided.

Step 2: Download and Accomplish the Enrollment Form

Parents/guardians may download the Enrollment Form directly from the school website.
Simply go to the Downloads section and look for the Enrollment Form.

Once downloaded, kindly complete the form accurately. The accomplished form should be submitted together with the required enrollment documents in person at the Registrar’s Office.

Step 3: Submission of Enrollment Requirements

Submit the completed Enrollment Form along with the required documents for the child’s grade level, in accordance with the guidelines of the Department of Education.

Incomplete requirements may be accepted temporarily, subject to submission deadlines set by the school.

Step 4: Assessment and Evaluation

The Registrar’s Office reviews all submitted documents to confirm:

• eligibility for the intended grade level

• completion of previous school requirements

For certain levels, such as Preschool or Senior High School, an interview or simple assessment may be conducted.

For students with special educational needs, they are required to undergo a developmental assessment or must submit an updated developmental assessment result at the office.

Step 5: Confirmation of Grade Level or Track

Once evaluated:

• Preschool to Junior High School students is confirmed for the appropriate grade level.

• Senior High School students confirm their chosen track and strand, subject to availability.

Step 6: Payment of Enrollment Fees

Parents/guardians proceed to the Cashiers to settle the required enrollment fees.

Official receipts will be issued upon payment. Available payment options or schedules will be explained by the school.

Step 7: Enrollment Confirmation

After payment and verification of documents, the student’s enrollment is officially confirmed. An Enrollment Confirmation Slip or acknowledgment will be issued for further notice.

Step 8: Release of School Information

Parents and students will receive important school details, including:

• class section and schedule

• school calendar

• uniform guidelines

• school policies and orientation schedule

• Enrollment is considered complete only after payment and document verification.

• Transfer credentials (SF10) are usually forwarded directly by the previous school.

• Please observe deadlines for submission of pending documents.
